- On or about March 22, 2024, the Alleged Victim (AV) was diagnosed with a urinary tract infection and prescribed antibiotics. Staff did not administer all of AV's medication due to AV's medication being in a bottle rather than a bubble pack and marked the medication unavailable. The error was discovered, and AV's medications were continued, however, they were delayed, placing AV at risk for serious harm. The facility failed to provide a safe medication administration system, resulting in a risk of serious harm. These actions constitute abuse by neglect as defined by OAR 411-020-0002(1)(b)(A)(ii)
