Findings
The Alleged Victim (AV) relies on the facility for his/her care and had a significant change of condition relating to falls. According to an investigation, AV had multiple documented falls between February 08, 2024, and April 20, 2024, some resulting in injury. There were interventions noted on facility incident reports, but they were never added to AV’s Service Plan. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
