- On or about April 19, 2024, Alleged Perpetrator #2 (AP2) was assisting the Alleged Victim (AV) with a transfer using a gait belt. AV is a two person assist with a gait belt for transfers. AP2 placed the gait belt across AV's chest and used one hand on the gait belt to assist AV. AV complained it was too much pressure on the belt assisting AV. A couple days later, AV complained of chest pain and shortness of breath and was sent to the ER. It was determined that AV suffered from bruising to the chest area. AP2's failure to properly assist AV with transfer, causing bruising, is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to ensure staff are properly executing care is a violation of Oregon Administrative Rules.
