Findings
The Alleged Victim (AV) is known to propel his/her wheelchair using his/her feet, which can cause wounds to AV's feet, and he/she is to have footwear on to ensure no injury. As well, the way that AV self-propels, he/she puts self at risk of slipping out of the wheelchair. On or about April 16, 2024, AV fell from his/her wheelchair and suffered a skin tear. AV fell from the wheelchair again on April 17, 2024, from getting his/her feet caught in the wheels. Respondent failed to properly care plan for AV's safety. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
