Findings
On or about April 20, 2024, Alleged Victim (AV) unsafely left the facility without assistance and was exposed to potential harm but did not suffer any injury while out of the facility. Based on facility documentation and interviews, AV left facility and returned on his/her own and was able to exit the facility due to the gate not locking or latching properly which allowed AV to simply walk out of the facility. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
