Findings
According to the documentation, the Alleged Perpetrator 2 (AP2) failed to protect the Alleged Victim (AV) from physical abuse. The AV reported on numerous occasion the AP2 intentionally throwing candy at their head and hitting them and hitting their legs knowing the AV has a diagnosis that may contribute to leg pain, which is a violation of resident rights and is considered abuse. The facility failed to provide a safe environment which is a violation of Oregon administrative rules.
