Findings
The Alleged Victim (AV) moved into the facility on April 10, 2024. AV had medications that he/she was supposed to be receiving. The facility failed to ensure AV's medications were approved in the medication administration system, which caused AV not to receive his/her medication for four days. The facility's failure is a violation of Oregon Administrative Rules.
