Findings
The Alleged Victim (AV) had a physician appointment on or about April 24, 2024. It was thought that AV's adult child was meeting AV at the appointment. AV was in the lobby of the physician's office before anyone noticed him/her. The facility was called and sent a staff to go get AV. Miscommunication between the facility and AV's child left AV at risk for harm, by being left at the physician's office without supervision. The facility's failure is a violation of Oregon Administrative Rules.
