Findings
The facility failed to provide a safe medication administration system to ensure the Alleged Victim's (AV) medication was administered as ordered. The failure resulted in AV going without multiple doses of multiple medications creating a higher risk of falls. AV experienced a non-witnessed fall resulting in skin tears and increased pain. The failures are a violation of resident rights, are considered neglect of care and constitute abuse.
