- The Alleged Victim (AV) relies on the facility for his/her care and is documented as a fall risk. According to an investigation, AV had a history of repeated falls, some with significant injury. On or about March 9, 2024, AV had a witnessed fall resulting in bruising and scratches to AV’s arms and leg. The facility failed to implement effective interventions to mitigate AV’s increasing fall risk and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
