Findings
The Alleged Victim (AV) relies on the facility for his/her care. The AV has a history of falls, self-transferring, and not being able to use his/her call button. AV was care planned for facility staff to clip the call light chord to AV’s clothing to alert staff when AV would stand up. According to an investigation, on or about April 3, 2024, AV was found lying on the floor, resulting in a hip fracture. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
