- The Alleged Victim (AV) relies on the facility for his/her care. AV is care planned for full assistance, including physical and verbal assistance with toileting needs. According to an investigation, on or about, May 6, 2024, AV was found in a soiled brief with dried feces and feces was found on AV’s floor and bed. According to documentation, within the previous thirty days, AV had developed a pressure ulcer to the buttocks and was found to have feces, urine, trash and/or old food in his/her room approximately fifteen times. The facility failed to provide appropriate services, relating to care planning, and following the care plan, posing a serious risk of harm and lack of personal dignity, which is a violation of resident rights, is neglect of care which constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) neglected AV was investigated and determined to be not substantiated.
