Findings
The Alleged Victim (AV) relies on the facility for his/her care. Witness 3 (W3) has a history of aggressive behaviors towards staff and other residents and is care planned to have two staff assist with care. According to an investigation, on or about May 6, 2024, AV and W3 were fighting over possession of a blanket. After being re-directed by a staff, W3 then reapproached AV and were fighting again over the blanket, resulting in AV having a skin tear and large bruise to the forearm. The facility failed to provide interventions in the service plan for staff to follow when aggressive behaviors occurred, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
