- Alleged Victim (AV) and Witness 1 (W1) have a known history of being involved in other resident-to-resident altercations. On or about May 16, 2024, AV walked into W1’s apartment. W1 stopped AV at the doorway and when AV went to leave, W1 grabbed a metal paper towel holder and hit AV in the face. AV then pushed W1 knocking W1 to the ground. AV and W1 have had previous altercations on or about March 20th and 26th, 2024. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
