Findings
The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, AV had a history of sliding to the floor from a seated position and was care planned to have the wheelchair footrests removed to help prevent injury, if they slide to the ground. On or about May 20, 2024, AV slid out of their wheelchair and was found sitting between the footrests with abrasions and scrapes. The facility failed to follow the care plan and interventions put in place to mitigate AV’s increasing fall risk and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
