Findings
The Alleged Victim (AV) has a history of behaviors and has a behavioral service plan in place to assist staff with managing AV's behaviors. On or about May 22, 2024, the Alleged Perpetrator #2 (AP2) yelled at AV using profanity. AP2 did not follow AV's service plan during this interaction with AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility failed to keep AV safe from verbal abuse, which is a violation of Oregon Administrative Rules.
