Findings
According to the documentation, the facility failed to follow the care plan for Witness 7 (W7) by not monitoring them for their known behavior of wandering and getting in resident-to-resident altercations. On or about May 23, 2024, W7 was witnessed leaving the Alleged Victim’s (AV) room. The AV was found on the floor and expressed pain on their head due to the resident to resident altercation. The facility staff admitted they could not account for W7 as outlined in their care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
