Findings
On or about May 27, 2024, Alleged Victim (AV) eloped from the secured facility and was found in the parking lot by facility staff. Based on facility documentation and interviews, AV unsafely left the facility without assistance and was exposed to potential harm but did not suffer any injury while out of the facility. The facility's failure is a violation of Oregon Administrative Rules.
