Findings
The Alleged Victim (AV) is a two person transfer. On or about May 30, 2024, Alleged Perpetrator #2 (AP2) went to AV's room to get him/her to come down for a vaccine clinic. AP2 requested assistance from other staff, however, they did not assist. AP2 transferred AV alone and AV suffered a skin tear. AP2's actions did not result in abuse. The facility's failure to ensure that AV's care plan was followed is a violation of resident rights, is considered neglect of care and constitutes abuse.
