Findings
On or about May 25, 2024, Alleged Victim (AV) did not receive his/her morning dose of medication prescribed to treat his/her seizure order. Based on facility documentation and interviews, Alleged Perpetrator 2 (AP2) was not properly trained by facility which caused this error. AV did not have any adverse effects from missing his/her morning dose. The facility failed to provide adequate staff training which resulted in AP2 marking AV's dose of seizure medication as given when it was not administered to AV. The facility's failure to provide a safe medication administration system is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no abuse occurred by AP2.
