Findings
The Alleged Victim (AV) relies on the facility for his/her care. The AV has a history of falls, self-transferring, and not being able to use his/her call button. According to an investigation between May 30, 2024 and June 1, 2024, AV experienced multiple falls, some with injury, causing repeated unreasonable discomfort. The interventions put in place after the falls were repeated and already in place on the Service Plan. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s ongoing falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
