Findings
The Alleged Perpetrator 2 (AP2) used physical force by grabbing the Alleged Victim's (AV) arm and pushing him/her into their room causing bruising to him/her arm. AP2's actions are considered physical abuse. The facility failed to provide a safe environment by failing to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
