Findings
The facility failed to provide appropriate oversight and monitoring when the Alleged Victim's (AV) condition changed which resulted in AV's condition worsening, causing AV being sent to the hospital for treatment. Based on facility documentation and interviews, the facility failed to provide the basic care of nutrition and fluids to AV which caused AV to suffer dramatic weight loss, confusion, and weakness. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
