Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about June 2, 2024, feces was found on AV’s forehead. It is believed Alleged Perpetrator 2 (AP2) was the individual who smeared feces on AV since he/she was working the day that AV had feces place on his/her forehead. In addition, staff recorded AP2 wiping feces on a dryer on the facility as well as holding gloves full of feces. AP2 appeared to have feces on his/her hands, as well as tucking a glove of feces in his/her shirt pocket. AP2 failed to protect AV from emotional abuse by placing feces on AV’s forehead, which neglect of care and constitutes abuse. The facility failed to protect AV from emotional abuse, which is a violation of Oregon Administrative Rules.
