Findings
The Alleged Victim (AV) is legally blind and relies on the facility to meet his/her daily needs. According to an investigation, on or about June 6, 2024, the Alleged Perpetrator 2 (AP2) failed to provide a safe environment when he/she left a skateboard unattended in the hallway of the residential unit, resulting in AV tripping and falling on the skateboard and fracturing his/her thumb. AP2's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment, which violates Oregon Administrative Rules.
