Findings
The Alleged Victim (AV) relies on the facility to meet his/her daily needs. According to an investigation, on or about January 29, 2024, the Alleged Perpetrator 2 (AP2) restrained AV by giving a "bear hug" while providing a brief change, resulting in AV becoming agitated. AP2's actions are a violation of resident's rights, are considered abuse and constitutes wrongful use of a restraint. The facility failed to protect AV from wrongful restraint, which is a violation of Oregon Administrative Rules.
