Findings
The Alleged Victim (AV) relies on the facility for care and is a full assist with teeth brushing and all hygiene needs. According to an investigation, on or about June 10, 2024, it was reported that AV’s top teeth were in shards and AV’s bottom teeth were black, grey and cracking. The lack of dental care assistance by the facility resulted in unreasonable discomfort and a loss of personal dignity to the AV. The facility failed to provide oral hygiene services for the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
