- On or about June 13, 2024, it was discovered that the Alleged Victim (AV) had bruising in his/her chest area. AV is a transfer with a gait belt, and the bruising was consistent with the improper use of a gait belt. All staff at the facility were not properly trained on proper placement of the gait belt and were placing the gait belt under AV's chest. Due to the misplacement and improper training, AV suffered a bruise and risk for harm. The facility's failure to properly train staff placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
