Findings
The Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has a history of behaviors and physical altercations. According to an investigation, on or about June 22, 2024, W1 slapped AV on the hand and AV yelled out in pain. W1 then slapped AV on a hand a second time, prior to facility staff intervening. AV was later noted to have bruising to his/her hand from the altercations. The facility failed to provide services to keep AV safe, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
