Findings
The Alleged Victim (AV) relies on the facility to provide care. According to an investigation, on or about June 24, 2025, AV was seen at a clinic for wound care services. It was noted by the medical staff that AV's wounds are not healing, and this is due to the facility not following AV's care plan for incontinence changes, wound care and nursing follow up at the facility. The facility failed to follow the care plan for AV, which is a violation of resident rights, and considered neglect with constitutes abuse.
