Findings
The Alleged Victim (AV) is scheduled to receive two showers per week, however, AV is only receiving one shower per week. At times, AV will refuse showers, however, the facility is sometimes short staffed, not allowing for AV to have his/her shower twice a week. The facility's failure to ensure proper staffing is a violation of Oregon Administrative Rules.
