Findings
According to the documentation, the facility failed to follow the care plan for Witness 1’s (W1) known inappropriate behavior. On the most recent service plan update for W1 on or about June 12, 2024, and a temporary service plan update on or about June 20, 2024, provided direction to staff to monitor W1’s behavior for sexually inappropriate interactions. W1 has mistaken a couple residents in the facility as their spouse. On or about June 29, 2024, W1 was expressing behaviors and then kissed the Alleged Victim (AV) on the cheek. Staff were present in the day room but failed to intervene to prevent the incident, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
