- According to the documentation, an unknown perpetrator (AP2) took the Alleged Victim’s (AV) credit card and used it to make unauthorized purchases. From approximately May 21, 2024, through June 14, 2024, AP2 made approximately seven unauthorized purchases. There were numerous care staff from the facility and outside agencies who had access to the AV’s room. It is unknown currently who the perpetrator is. The AP2 took AV’s bank card without permission, which is a violation of resident rights, is considered neglect of care and constitutes financial abuse. The facility failed to protect the AV from financial abuse which is a violation of Oregon Administrative Rules.
