Findings
The Alleged Victim (AV) relies on the facility to meet his/her daily needs. According to an investigation, on or about June 25, 2024, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) failed to follow the care plan when they physically restrained AV's arms during care and did not leave AV alone while monitoring for safety when AV became resistive to care. AP2 and AP3's actions resulted in AV sustaining bruises and unreasonable discomfort. AP2 and AP3's actions are considered physical abuse and constitutes abuse. The facility failed to ensure the service plan was followed, which violates Oregon Administrative Rules.
