- The facility failed to provide appropriate services according to the Alleged Victim’s (AV) wound care needs. On or about July 8, 2024, AV arrived at his/her wound care appointment observed to be wearing the same bandages from his/her last wound care appointment which was on June 11, 2024. When AV's wound dressing was removed, maggots were observed inside the wound and also observed to fall off of AV's leg. A foul smell came from AV's wound. Based on facility documentation and interviews, the last time AV's leg wraps had been changed was on June 11, 2024, at AV's last wound care appointment. AV suffered unreasonable discomfort and pain when the bandages were removed due to the bandages being embedded in AV's skin. The failure resulted in AV’s condition worsening causing further unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
