Findings
The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of wandering on the unit. According to an investigation, on or about July 4, 2024, AV complained of pain and had discoloration and swelling in the collarbone area. AV's service planned interventions include for AV to be escorted to/from activities and the dining room. Prior to the injuries observed, AV was walking around the common area and attending an activity. Another resident told staff that AV fell in the bathroom. The facility failed to follow AV's service plan and escort AV to/from the activity in the common area provide supervision, resulting in AV's unwitnessed fall and fractured collarbone. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
