- The Alleged Victim (AV) relies on the facility for his/her care. Fall interventions documented in AV's care plan include use of a pressure pad alarm when AV is sitting in recliner. Staff are instructed to ensure the alarm pad is in place and working correctly at all times. According to an investigation, on or about July 10, 2024, AV was found on the floor near their recliner, resulting in laceration to the eyebrow area and requiring treatment at the hospital. The fall intervention of placing the pressure pad alarm on the recliner and ensuring it was functioning had not been provided prior to the fall. The facility failed to follow the AV's care plan, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
