Findings
The Alleged Victim (AV) relies on the facility for his/her care and is known to have exit seeking behavior. According to an investigation, on or about July 10, 2024, AV eloped out an unsecured door that maintenance workers had been using, posing a risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
