Findings
The Alleged Victim (AV) is a known fall risk. On or about June 19, 2024, AV suffered an unwitnessed fall out of bed and hit his/her head. AV refused to go to the hospital but complained of pain in his/her hip and rib area, and had a bruise on his/her hip. AV's care plan does not have any interventions in place to reduce AV's risk of falls, before or after this fall. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
