Findings
The Alleged Victim (AV) relies on the facility for care, had a recent medication change and was on alert with instructions for staff to watch for an increase in drowsiness, dizziness and poor coordination. According to documentation staff observed an increase in drowsiness and poor coordination, AV was found sleeping in common areas of the facility on multiple occasions, and AV needed assistance to walk and transfer on July 19, 2024 and July 20, 2024. On or about July 23, 2024, AV was found on the floor of h/h bathroom, apparently having fallen from h/h toilet, resulting in a bump on AVs head. The facility failed to provide appropriate services according to AV’s needs, regarding care planning, safety checks and monitoring, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
