Findings
On or about August 13, 2024, the Alleged Victim (AV) eloped through an open gate in the facility courtyard. AV has eloped through the gate in the courtyard multiple times. The facility knew that the gate didn't latch properly and failed to have the gate fixed to ensure resident safety while in the courtyard. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
