Findings
The Alleged Victim (AV) relies on the facility for care and to provide a safe environment. According to an investigation, AV suffered five falls from June 28, 2024, through August 3, 2024, with the last fall resulting in a wrist fracture. The facility failed to implement progressive measures to prevent AV from suffering repeated falls and did not provide a safe environment for AV. This is a violation of resident rights, is neglect of care and constitutes abuse.
