Findings
On or about August 3, 2024, the Alleged Victim (AV) suffered a fall and was sent to the hospital. Upon AV's return, AV had a change of condition, needing a higher level of care. The facility failed to complete a change of condition assessment to address AV's change in care needs. Failure to provide a change of condition assessment is a violation of Oregon Administrative Rules.
