- The Alleged Victim (AV) and Witness #1 (W1) have a history of altercations. On or about August 17, 2024, W1 wandered into AV's room and both residents began to hit each other. Staff were able to separate the two and remove W1 from AV's room. Failure to have appropriate interventions placed both residents at risk for harm. The facility's failure to ensure resident safety from altercations is a violation of resident rights, is considered neglect of care and constitutes abuse.
