- The Alleged Victim (AV) relies on the facility for his/her care and has a history of leaving the facility with some difficulty returning safely. Interventions documented in AV's care plan included to remind AV it is unsafe to leave the facility alone and to ask staff for help. Other interventions provided, but not listed in AV's service plan, were for staff to provide safety checks every two hours. Following another incident on June 7, 2024 when AV left the facility alone and needed redirection to return, facility staff documented AV having confusion and declining memory during evening hours. No interventions were implemented to address the behavior. According to an investigation, on or about August 19, 2024, AV fell in the parking lot outside the facility after leaving alone, resulting in a laceration to the head and a fractured elbow. The facility failed to plan care, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
