Findings
The Alleged Victim (AV) is a known fall risk and is care planned for assistance with ambulation, toileting reminders during safety checks, and reminders to use his/her walker. On or about August 20, 2024, AV was found on the floor of his/her room after a fall trying to utilize his/her commode. AV was transferred to the hospital and diagnosed with a broken finger. AV had just had a safety check and was not asked if they needed to utilize the commode as outlined in the care plan. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
