- Alleged Victim (AV) and Witness 1 (W1) have a history of being involved in resident-to-resident altercations, and have a history of aggressive behaviors. W1 has a history of wandering into other resident rooms. AV does not like other residents entering into AV's room. On or about August 21, 2024, W1 entered into AV's room. AV was found on top of W1, hitting W1. W1 was observed to be hitting AV back. W1 was last seen by staff walking around the memory care unit prior to entering AV's room. W1's service plan does not have listed interventions to address W1 wandering into other resident's room. As a result of the resident-to-resident altercation, AV sustained a cut, approximately 1 inch in length to AV's right ring finger. Additionally, AV presented as very emotional following this altercation with W1, requiring a PRN medication for emotional distress. The Facility failed to provide a safe environment for AV and the facility failed to appropriately adjust W1 care plan and implement reasonable interventions to address W1s continued wandering into other resident rooms, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
