- The Alleged Victim (AV) relies on the facility for his or her safety. AV’s Service Plan states that AV has a history of exit seeking and needs to be monitored and redirected away from doors. According to an investigation, on or about August 24, 2024, AV was given a shower at approximately 10:30 am. At around 12:15 pm, facility staff could not locate AV. Facility staff began searching the facility and surrounding areas, as the facility is in close proximity to a 35 mph, busy and heavily traveled five lane road. During the search of the building and perimeter a large hole, approximately 5-foot high, was found in the secure outdoor area fencing. AV was ultimately found at an outside event for a neighboring church and was brought back to the facility by staff. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse
