Findings
The Alleged Victim (AV) moved into the facility on August 23, 2024, and had eloped from a previous residence. Between August 25, 2024, and August 28, 2024, AV was actively exit seeking and did elope from the facility twice in one day. During the elopement, AV fell from his/her wheelchair three times. The respondent's failure to provide a safe environment and prevent AV from having falls placed AV at risk for harm.
