Findings
The Alleged Victim (AV) has a chronic skin condition and relies on the facility to manage their care, including skin care. According to an investigation, on or about August 18, 2024, the Alleged Perpetrator 2 (AP2) was rough while providing skin care to the AV, causing discomfort, an open wound, and bleeding. AP2’s actions are a violation of resident rights, are considered neglect of care and constitutes abuse. The facility failed to keep AV free from rough treatment during care, which is a violation of Oregon Administrative rules.
